Complete the application below to request a new license, apply to change your ministry position, renew an existing license, or request a replacement. Provide accurate information and upload a clear, recent applicant photo.
Before you begin:

- Review all information carefully before selecting Submit Form.
- Honorary licenses: Honorary Pastor and Honorary Bishop license applications do not require a fee.
- All other license applications: A $100 license fee applies to Minister, Deacon, Pastor, and Bishop licenses for New, Change Ministry Position, Renewal, and Replacement applications. After submitting the form, you will be redirected to our secure payment page.

Frequently asked questions
How much does a license cost?
Honorary Pastor and Honorary Bishop license applications are free. Minister, Deacon, Pastor, and Bishop license applications require a $100 license fee for New, Change Ministry Position, Renewal, and Replacement applications. Paid applications will be redirected to our secure payment page after submitting the form.
How long does license processing take?
Complete applications are normally reviewed, prepared, and mailed within 5–10 business days after all required information, the applicant photo, and any applicable payment have been received. This timeframe ends when the license is mailed; postal delivery time is additional and may vary.
What happens after I submit my application?
Honorary Pastor and Honorary Bishop applicants will receive an on-screen confirmation and will not be redirected to payment. Applicants for Minister, Deacon, Pastor, and Bishop licenses will be redirected to the secure payment page after submitting the form. Our office will then review the completed application.
How will I receive my license?
Once approved, your physical license will be mailed to the destination selected in your application: your home address, your church address, or a different mailing address. Please make sure the selected address is complete and accurate.
